Fix Minor Bugs

- Removes Others option from department, title and how_did_you_hear_about_us field
- Removes placeholders in login form(template)
- Fixes testimonial bug for mobile view
- Adjust padding and font size for title statement in index page
- Adds How to Participate in view_workshop_details page
